We are seeking a dedicated and experienced Band 6 Senior Nurse to join our Locality Co-ordination Centre team in North and West Bristol. This role is crucial in supporting the smooth delivery and coordination of patient care across the locality. As a Senior Nurse, you will be working closely with healthcare teams to manage patient care processes, improve service efficiency, and ensure high standards of patient outcomes. This role offers an excellent opportunity to work in a supportive, multidisciplinary environment where your skills and insights will play a vital role in enhancing patient care for the community.

Job Description

n this role, you will coordinate and oversee patient care across North and West Bristol, ensuring that each patient receives timely and effective care through well-organised healthcare services. Your responsibilities will include liaising with various healthcare teams, utilising your problem-solving skills to support service delivery, and proactively managing the coordination of patient services. This is a dynamic position requiring a blend of strong organisational abilities, clinical knowledge, and a patient-focused approach to deliver the best outcomes.

Key Responsibilities:

Coordinate patient care across the North and West Bristol region, ensuring efficient management of resources.
Collaborate with multidisciplinary healthcare teams to deliver integrated and effective service.
Provide support, guidance, and problem-solving to optimise patient care processes.
Monitor and manage patient service delivery to maintain high standards of care.

Requirements

Band 6 Nursing Qualification: A current NMC registration and Band 6 experience are essential for this role.
Coordination Experience: Prior experience in care coordination or a similar nursing role is preferred, with a demonstrated ability to manage patient care effectively.
Communication and Organisational Skills: Exceptional organisational abilities, along with strong written and verbal communication skills, to liaise with healthcare teams and ensure clarity in patient care processes.
Team Player: Ability to work both independently and collaboratively within a multidisciplinary team, providing support and promoting a cohesive team environment.
Problem-Solving Skills: A proactive approach to problem-solving, with the ability to manage challenges in patient care coordination.
